  1. At the top right of this page, click Duplicate (the square icon) to copy the entire template to your own Notion workspace.
  2. Open your copy, then click New in any database to add your first entry. Sample rows are already included to show you what a complete entry looks like.
  3. Track daily for two to four weeks. Most patterns become visible in the first month.
  4. Before a clinician visit, filter the database to the last 30 days and either print or share the Notion link with your provider.
  5. Your data stays in your Notion workspace. Nothing is sent back to us.
